Job Title: Nurse Practitioner (NP)
Position Summary
The Nurse Practitioner (NP) works collaboratively with a dedicated clinical care team to deliver evidence-based primary and family healthcare services to patients. The provider will be responsible for assessment, diagnosis, treatment, health counseling, chronic disease management, and disposition planning while helping improve overall patient outcomes. This position reports directly to the Practice Manager.
Note: This is a safety-sensitive position.
Key Responsibilities
Clinical Duties
- Provide comprehensive primary and family care services, including:
- Patient assessment and diagnosis
- Treatment planning and management
- Health counseling and patient education
- Chronic disease management
- Preventive care and wellness services
- Disposition planning and referrals as needed
- Conduct routine physical examinations and diagnostic evaluations.
- Develop and maintain comprehensive patient care plans in collaboration with other healthcare professionals.
- Encourage patient engagement in their own healthcare through health screenings, wellness initiatives, and behavioral health support.
Administrative Duties
- Open and close clinic operations as required.
- Manage clinic inventory and supplies.
- Verify patient insurance information and collect payments.
- Conduct patient follow-up communications.
- Maintain a clean, safe, and compliant clinical environment.
- Accurately document patient encounters using Electronic Medical Systems (EMS).
Required Qualifications
Experience:
- Minimum of 1 year of recent Nurse Practitioner experience in:
- Primary Care
- Family Medicine
- Urgent Care
- Candidates without prior Primary Care or Urgent Care experience will not be considered.
- Strong understanding of quality measures and chronic disease management.
Education:
- Master's Degree from an accredited Family Nurse Practitioner Program.
- Current National Board Certification as a Family Nurse Practitioner.
Licensure & Certifications:
- Active Nurse Practitioner license in the state of employment.
- License must be in good standing with no current or prior disciplinary actions.
- Current Basic Life Support (BLS) certification from:
- American Heart Association (AHA), or
- American Red Cross (ARC)
- DEA registration (or willingness to obtain during onboarding, if required).
- Willingness to obtain additional state licensure as needed.

Work Schedule
- Training: Monday–Friday, 8-hour shifts for 2 weeks.
- Regular Schedule: 4 days per week, 10-hour shifts.
- Requires working every other weekend.
- Holiday rotation may be required.
Work Environment
- In-person, patient-facing role.
- Onsite attendance required as scheduled.
- Ability to work independently while maintaining collaboration with the broader care team.
